I propose to discuss management of near drowning in sea water in Israel, in particular addressing the prevention of AKI following such an episode.

We are pioneers in this field and there are no available protocols.
In the near future a follow-up review in this issue is to appear in Nephrol Dial Transplant, where we propose to attenuate oxygen expenditure for tubular transport by medullary thick limbs with furosemide (20 mg for an adult).
Given the opportunity I can extend on the rationale for this treatment in an effort to embark upon a national treatment protocol to be adopted and tested.
Being located in seaside Tel Aviv, we treat many near drowning victims each summer. AKI is very rare, is usually part of a multi system anoxic organ injury syndrome and has in my view nothing to do with seawater per se.
The AKI is usually mild and resolves with all other anoxic organ injuries, and it is rarely in itself of importance. Rhabdomyolysis is not uncommon and may contribute to AKI, but again it is rarely the life-threatening event.
